Agency in Kinston, NC is seeking a dedicated Licensed Practical Nurse to join their patient-centered team. Position Summary
You will be a vital link, working alongside physicians, nurses, and medical assistants to deliver timely and effective patient care. You will utilize your full LPN scope of practice to conduct appointments, assist with complex cases, and help execute comprehensive care plans. This role combines clinical, administrative, and patient education duties, contributing to a robust, team-based healthcare model focused on serving our community.
Key Responsibilities
• Conduct patient intake, including medical histories, agenda setting, and performing basic clinical measurements.
• Assist with Annual Wellness Exams, screenings, medication reconciliation, and identifying care gaps.
• Provide patient education and follow-up care within the LPN scope.
• Collect and prepare lab specimens; ensure proper instrument sterilization and disposal of contaminated supplies.
• Respond to patient phone calls and provide triage support.
• Work independently within scope, using sound clinical judgment.
Care Coordination & Team-Based Care
• Actively support care coordination efforts by collaborating with clinical staff, Health Educators, community health workers, and external partners (including organizations like NC Field and the American Heart Association).
• Work with the staff to ensure mobile medical unit grant compliance.
Documentation & Administrative Support
• Accurately document patient interactions and services in the Electronic Health Record (EHR) system.
• Assist with data entry and tracking key performance measures for quality assurance and grant purposes.
Community Engagement
• Attend and support community events, representing agency and assisting with setup/breakdown.
• Maintain current knowledge of available community resources and refer patients appropriately.
Job Requirements
Qualifications:
• Education: Graduate of an accredited practical nursing program.
• Certifications & Licenses: Valid LPN license in the state of North Carolina and current BLS certification.
• Experience: 2–5 years of nursing experience is preferred.
• Skills: Proficiency with EHR systems and Microsoft Office Suite.
Additional Information
Schedule Monday - Friday, 8:00 a.m. - 5:00 p.m.
